Understanding Knee Pain: Dr Bayes’ Approach to the “Unhappy Knee”

Knee pain is one of the most common reasons patients seek medical attention. It can develop suddenly after an injury or gradually over time, affecting mobility, daily activities, and overall quality of life.

According to Dr Bayes, diagnosing knee pain begins with three key steps: a detailed patient history, a focused examination, and the correct investigations. Although the knee is a complex joint, most painful knees can be grouped into a few main categories.

The Four Types of “Unhappy Knees”

Dr Bayes generally categorises knee pain into four groups:

  1. Arthritic conditions
  2. Sports or ligament injuries
  3. Overuse injuries
  4. Red-flag conditions (rare but serious)

Pain is the most common complaint across all these groups, but the cause often differs depending on age, activity level, and the nature of the symptoms. For example, younger and active patients frequently experience sports-related injuries, while older patients are more likely to develop degenerative conditions such as Osteoarthritis.

A key indicator when assessing knee injuries is how quickly swelling develops after trauma.

Rapid Swelling

If swelling occurs within a few hours of injury, it often indicates bleeding inside the joint. This can be caused by:

  • Ligament injuries such as ACL tears
  • Fractures around the knee
  • Patella dislocation
  • Major meniscus tears

These injuries usually make it difficult or impossible for the patient to continue normal activity.

Delayed Swelling

Swelling that develops over a day or more often results from irritation inside the joint. Common causes include:

  • Meniscus injuries
  • Cartilage damage
  • Ligament sprains

Meniscus injuries may cause pain along the joint line, clicking, or difficulty fully straightening the knee. Because the meniscus has limited blood supply, early evaluation can be important.

Non-Traumatic Knee Problems

Not all knee pain is caused by injury. Some cases are related to underlying medical conditions that cause inflammation or degeneration in the joint.

Examples include:

  • Gout
  • Inflammatory Arthritis
  • Osteoarthritis

These conditions can cause swelling, stiffness, and discomfort due to increased fluid production within the joint.

When Knee Pain Is a Medical Emergency

Although uncommon, certain knee conditions require urgent treatment. One of the most serious is Septic Arthritis, an infection inside the joint.

Symptoms may include severe pain, fever, rapid swelling, and difficulty bearing weight. Without prompt treatment, the infection can cause rapid joint damage and serious complications.

If further evaluation is needed, doctors may recommend imaging tests such as X-rays, ultrasound, or MRI scans to better understand the underlying problem.

Treatment depends on the diagnosis but may include physiotherapy, anti-inflammatory medication, joint injections, or lifestyle and activity modification.

In more advanced cases, surgical procedures such as arthroscopy or knee replacement surgery may be considered to relieve pain and restore function.
